New Patient Information
When you call to make an appointment you will be asked to provide:
- your name(s)
- date of birth
- type of insurance
- a phone number where we may leave messages
- If calling for a minor, please indicate that you are the parent or guardian and give us your contact information as well.
When you come in for your appointment you must have with you:
- your insurance card
- driver’s license
- If you have a co-pay, it is due at that time.
When you come in for your scheduled appointment please bring with you the following:
- Driver’s License
- Insurance Card(s)
- Copay, if applicable
- In the case of a referral, documentation or records from referring physician unless they have been sent previously to the office for doctor’s review
